Yes. Some patients may be able to switch from a Wegovy injection to the Wegovy pill or from the Wegovy pill to an injection following a prescriber review.
Both treatments contain semaglutide, but they are absorbed differently and are not directly interchangeable on a milligram-for-milligram basis. Wegovy injections are taken once weekly, whereas the Wegovy pill is taken once daily and must be taken according to specific fasting, food and water instructions.
Switching from a Wegovy injection to the Wegovy pill
| Current Wegovy injection dose | Estimated Wegovy pill switching dose |
|---|---|
| 0.25 mg once weekly | 4 mg once daily |
| 0.5 mg once weekly | 9 mg once daily |
| 1 mg once weekly | 9 mg once daily |
| 1.7 mg once weekly | 9 mg once daily |
| 2.4 mg once weekly | 25 mg once daily |
| 7.2 mg once weekly | 25 mg once daily |
The Wegovy tablet prescribing information specifically states that patients treated with Wegovy 2.4 mg injections once weekly can transition to Wegovy 25 mg tablets once daily, starting one week after their final injection.
The other conversions in the table are Lotus’s estimated off-label switching guidance and have not been formally evaluated in direct clinical trials.
Switching from the Wegovy pill to a Wegovy injection
| Current Wegovy pill dose | Estimated Wegovy injection switching dose |
| 1.5 mg once daily | 0.25 mg once weekly |
| 4 mg once daily | 0.5 mg once weekly |
| 9 mg once daily | 1 mg once weekly |
| 25 mg once daily | 1.7 mg once weekly |
Switching from the Wegovy pill directly to a Wegovy injection above its usual 0.25 mg starting dose is an off-label approach. There is no manufacturer-approved dose-conversion table for switching in this direction.
Your prescriber will confirm your dose after reviewing your current treatment, when you last took it, how well you have tolerated it and whether you have experienced any side effects. A lower dose may be recommended if you have had side effects or a break in treatment.
Wegovy tablets and injections must not be used at the same time. Your prescriber will tell you when to take your final dose of your current treatment and when to begin the new one.
